Submit your Declaration Form
Families part of our Waiver Program must provide up-to-date information for their Waiver Payment via the Declaration Form.
Verify Your Form Information
You will be contacted by us to verify your Declaration Form.
Enroll & Attend Swimming Lessons
Once your Declaration Form is verified, you can enroll in lessons approved by your Service Coordinator.
You must attend 100% of your lessons. See below for more details.
How Our Program Works
All agencies and organizations must approve students prior to lessons beginning at FOSS.
Families must submit a quarterly form declaring their waiver payment prior to enrollment.
If a business wants to sponsor a swimmer for the quarter, we recommend purchasing a gift card rather than using the waiver payment process.
Frequently Asked Questions
To ensure a smooth invoicing process and communication throughout the quarter, families must provide up-to-date information for their waiver payment. This must be completed through our Declaration Form on our website prior to enrolling in lessons. This form will be required for all students interested in swimming for the upcoming quarter prior to enrolling at FOSS. This form can be submitted at any time prior to lessons beginning for the quarter they are interested in enrolling in.
Once we receive their Declaration Form, the Waiver Program Team will communicate and verify with the service coordinator and guardian listed on the form. Once FOSS has verified their waiver payment for the upcoming quarter from the service coordinator, the family is ready to enroll in lessons.
- Private Lessons are to have 100% attendance, we currently do not offer make-up lessons for students enrolled in Private Lessons. If a lesson is missed, the guardian is responsible for notifying FOSS in advance.
- Group Lessons are to have 100% attendance. If a lesson is missed, you must schedule a make-up lesson by the end of the quarter.
- If the family does not notify FOSS of missed lessons, FOSS has the ability to cancel enrollment for the remainder of the quarter and future enrollments at FOSS through the Waiver Payment Program.
- Anyone who has not submit a Declaration Form is ineligible for swimming in the upcoming quarter with a Waiver Payment.
- If the family does not notify FOSS of missed lessons, FOSS has the ability to cancel enrollment for the remainder of the quarter and future enrollments at FOSS through the Waiver Payment Program.
If your waiver payment does not cover your full tuition cost, the guardian is required to pay the outstanding balance.
- A separate form is required for each student.
- You are enrolling the named student in FOSS swim lessons.
- Enrollment is not guaranteed until funding has been reviewed and approved by your waiver agency.
- If your agency covers only part of the cost, you will be responsible for any remaining balance.
- If you have an unpaid balance or funding is not approved or received, we reserve the right to cancel your current enrollment and not allow you to enroll in future lessons.
- This form will be confidential and only used to receive payment from agency.
